How Intismeran autogene (V940) works

Intismeran autogene (V940, also called mRNA-4157) is an investigational individualized neoantigen therapy. It encodes selected targets based on the mutational signature of a patient's tumor. This example follows the vaccine and the separate contribution of pembrolizumab in melanoma after surgery.

  1. Select and encode. Tumor-specific changes inform the individualized mRNA design.
  2. Deliver, translate and present. Antigen-containing proteins are produced and processed so peptides can be presented to the immune system.
  3. Prime and recognize. Responsive T cells expand and can recognize matching peptide–MHC on a target cell.
  4. Block an inhibitory interaction. Pembrolizumab binds PD-1 and blocks ligand engagement; this is distinct from the vaccine's antigen instructions.
